OPT OUT REQUEST FORM: HERBICIDES (ONLINE VERSION)

Request for exclusion (opt out) from wide area right-of-way herbicide application. Any private property owner or individual in lawful possession of a property may request exclusion from wide area application of herbicides. This includes applications performed by Nolin Rural Electric Cooperative Corporation (Nolin RECC) or its subsidiary Wide Open Utility Service. Requests must be made to Nolin RECC or Wide Open Utility Service using this form and will go into effect fourteen (14) days from the date the request is made. Future requests for opting out must be presented to Nolin RECC or Wide Open Utility Service in the same manner as this request is made.
